Step 3: Post-Acceptance Forms

You’ve applied, been admitted, and now you’re coming to Florida College. Once you receive your acceptance letter, please read and complete every form in this section. All forms should be sent to:

Sign Your Enrollment Contract

Before you can register for your classes you will need to fill out an Enrollment Contract.

Parent Signature

Our admissions process requires a parent signature. Download the Parent Signature Form.

Housing Form and Security Deposit

Submitting your Housing Form and security deposit allows you to be placed in our residence halls. You can pay the security deposit online or send in a check. The off-campus fee is $100. The on-campus fee is $150. Please note: Once submitted, fifty dollars of the housing and security deposit is non-refundable.

Photo

You can either send in your senior photo or a head/shoulders shot. You can email it or mail it in.

Medical Forms A, B, and C

The State of Florida requires specific medical information on file for incoming students. Please try to make your physical appointment as early as you can within twelve months prior to enrollment. Freshmen physical appointments are sometimes hard to get if you wait until after high school graduation. Download the Medical Form.

ACT / SAT Scores

Please submit either ACT or SAT scores. These scores will be used for course placement and for determining eligibility for academic scholarships. Information on submitting your scores can be found here.

*Home Schooled Students

In addition to your standard application forms, we need you to submit a self-certification statement to help us comply with Federal and State financial aid guidelines.

Start Planning Your Classes

You can use the Florida College Catalog and class schedules to begin planning which courses you will take. If you’re in a degree program, some have strict guidelines about which courses you should take and when. Make sure you follow your degree program guidelines.

Get Ready for Opening Week

Lots of important things will be happening during opening week, including residence hall check-in, opening ceremonies, orientation, chorus and sports tryouts, games, devotionals, and more. Find the information you need concerning opening week.
