Application Fee

There is a $30 processing fee when you apply for admission. You can either mail the payment in with your application or pay your application fee online here.

Individual Application Forms

If you have already sent in the primary 2-page appliaction but still have not submitted some of the additional reference forms, you can print them out individually:

• The State of Florida requires all incoming students to submit certain medical records upon entrance to a Florida secondary educational institution. We have prepared three simple medical forms to make it easy for you to get everything done:
1. Medical Form A
• This form shows your inoculations and your physical examination. There are three vaccinations required by Florida for incoming students. You’ll need to be sure and have documented dates for the Hepatitis B (3-doses), MMR or Measles (2-doses) and the Meningococcal Meningitis (1-dose). You can take this form to your doctor when you’re ready for your physical.
2. Medical Form B
• This form is the parent authorization for emergency health care, health insurance information and emergency contact information. All students who attend Florida College are required to show proof of current health coverage.
3. Medical Form C
• You sign the top of this form to show that you have been provided and have read the inoculation information. You can also use this form to decline either the Hepatitis B or Meningococcal Meningitis vaccines (or both). This form must be signed at the top and submitted, even if you do not decline any vaccinations.

Home Schooled Students

If you are a home schooled student, make sure you also sign and send in a Self Certification Statement for Home Schools form:
